Model release form is a legal document that establishes a contract between photographer and
model. The model is protected by agreeing to how their image is used and the photographer has
legal permission to license the model’s photos.
By submitting this model release form

the Model gives the Photographer permission to use Model's images.

the Model agrees to the terms and conditions
of the photoshoot as of date of submitting.

the Model agrees to release the Photographer from any liability.

the Model and the Photographer have agreed compensation for a photoshoot.

the Model is of legal age or represented by a legal guardian.
